Kate M. Lally, MD, FACP, is the Chief of Palliative Care and Hospice Medical Director for Care New England and Medical Director for the Integra ACO. She is a graduate of Yale School of Medicine, and completed her post graduate training at the Hospital of the University of Pennsylvania. She serves on the faculty of the Warren Alpert Medical School of Brown University in the role of Assistant Professor of Medicine; Clinician Educator.
In her tenure at Care New England she has created robust interdisciplinary Palliative Care and Advanced Illness Management programs, and has substantially developed the hospice program. In addition she has also spearheaded Care New England’s involvement with IHI’s Conversation Ready Project. Along with the Palliative care team, she has developed the role of the “Conversations Nurse” to increase awareness about advanced care planning and patient focused goals of care across the state of Rhode Island. She currently serves as a faculty member at the IHI and is working nationally to educate other health care providers on how to become Conversation Ready.
